Project Description:

In collaboration with the state Division of Developmental Disabilities the AzUCD is implementing an innovative Uniform Assessment Tool (UAT) to facilitate determining the Level of Care for DDD members who are in need of and/or receiving active treatment in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), including DDD members residing at the Arizona Training Program in Coolidge (ATPC), State operated group homes.
