KCDD will work collaboratively with State agencies and other organizations to provide training support, and resources in areas of quality assurance for individuals with intellectual and developmental disabilities (IDD), their families, and support networks. Needs to be Addressed: Specifically the proposed project addresses the need in Kansas for self-advocates, family members, and professionals to receive training and technical assistance to access, enhance, and monitor supports and services that promote self-determination, independence, productivity, and integration and inclusion in all facets of community life (KCDD Five Year Plan, p. 22). The project will achieve this by coordinating a multi-level statewide structure for implementing and disseminating the Charting the LifeCourse (CtLC) framework and tools, building on existing CtLC and quality assurance initiatives in Kansas. Goal & Objectives: The overall goal of this project is to develop and sustain a statewide network of individuals with IDD, families, and professionals who can apply the Charting the LifeCourse framework and tools to access, enhance, and monitor integrated, person-centered supports and services for people with IDD and families across Kansas. Objective 1: Establish structure for a Charting the LifeCourse Statewide Network by creating a statewide Stakeholders Group and selecting three regional pilot networks. Objective 2: To conduct online Ambassadors Series trainings with three regional cohorts, with a total of 45 participants who will become certified CtLC Ambassadors. Objective 3: Coordinate and pilot three regional Tools in Action Workgroups that serve as learning collaboratives for the integration of person-centered practices into local supports and services. Intermediate outcomes: Increased capacity to engage stakeholders and integrate CtLC at state and community levels; Increased use of CtLC by professionals, families, and self-advocates in the pilot networks; Increased collaboration and shared learning among mixed stakeholders in pilot regions Long-term outcome: Improved access to appropriate, person-centered supports that promote self-determination, independence, productivity, and quality of life for individuals with IDD in Kansas
