Project Description:
Objective #3: DOD Support Coordinators will enhance knowledge and skills around DMH-DDD identified minimum standards of person centered planning to benefit the individuals receiving ODD services. Deliverable: Deliverable #1: UMKC will submit a work plan for this objective to include a timeline and schedule for deliverables per quarter-to be submitted September 1. 2018. Deliverable #2: DDD SC Leadership Series - 2-4 face to face events with Ambassador attendees and Executive Leadership, 3 Ambassadors Series (5 modules, 7 coaching calls) for SCs, SC Supervisors, RO ADs, RO Directors, TCM TACs, etc. (15 per cohort/series) (Eastern, Western, Central). Deliverable #3: Provide Technical Assistance to Statewide ODD SC Leads - Bi-weekly calls with Marcy/Carrie, 2 Face-to-Face meetings. Deliverable #4: Enhance/support current DOD Statewide Support Coordination policies, regulations and standards to provide consistent expectations and implementation across Missouri - Maintain and track utilization of SC Training Clearinghouse website, Identify and develop up to 3 tools aligned with SC Competencies (e.g. SC Self-Assessment, SC Performance Discussion Tool, SC Survey Crosswalk). Deliverable #5: Engage External Key Stakeholders Throughout Process - Host 4-6 calls with SC Advisory Group, Meet with MACDDS Board and/TCM group at least 4 times during year, Host At least 2 Learning Collaborative Meetings. Deliverable #6: Build the Capacity of Internal DDD Support Coordinators- DDD Support Coordinators will be trained using 6 1-hour long SC Core Foundational Competency On-Line Modules using training platform hosted by UMKC. IHD staff will assist DOD to develop 4-61-hour long SC Guidelines and Procedures On-Line Modules aligned with core competency using the Curriculum Development Guide and host within the training platform. Deliverable #7: Provide Technical Assistance for DDD Regional Offices - 6 1.5-hours SC Supervisor Calls, 4-6 1.5-hours All Support Coordinators Calls. Deliverable #8: Provide Targeted TA to St. Louis Regional Office - Form DDD STL RO SC Learning Collaborative focusing on PC/ISP Implementation, 61.5 hours TA Calls with StL SC Group, 2-day long TA/Training events. Deliverable #9: Provide TA on SC TAC Review Tools - Up to 6 1.5-hours TA Calls with TAC, TCM TAC Review Tool Guide, - SC Training Crosswalk/Submission Form and Guide.
