Project Description:
The SC Department of Health and Human Services has contracted with the SC UCEDD for a faculty member to manage the Aging and Disability Resource Center grant, SC Access Plus. This grant is one of the Real Choice grant jointly funded by the Administration on Aging and the Center for care and Medicaid Services. This grant builds on the first Real Choice Systems Change grant that developed a web-based information and referral system to enable persons with disabilities and their families to access information about services available to them (SC Access) and built a network of Information and Referral Specialists throughout the state. The primary objectives are: (1) to develop a coordinated Aging and Disability Resource Center that provides information to consumers, assists them to understand the information and make informed choices and to improved access to services and (2) improve the process by which consumers apply for Medicaid services.
