Project Description:
The overall goal of the project is to support individuals with intellectual disabilities to be included in and have access to the full range of evidenced-based physical activity and nutrition programs provided through public health programs within Missouri. IHD will Partner with DHSS to adapt public health programs to be accessible for people with ID, develop and disseminate project materials, and conduct evaluation.Project objectives include: 1. To develop an internal leadership infrastructure that supports state capacity to establish and sustain programmatic, policy, and systems improvements. 2. To enhance and expand partnerships between individuals with ID and their families, the agencies and organizations that serve them, and state public health programs. 3. To assess gaps in access to physical activity and nutrition public health programs and to disseminate resources and inclusion strategies. 4. To adapt and implement evidenced-based physical activity and nutrition programs for individuals with ID. 5. To evaluate program processes and outcomes and use results to support continuous program quality improvements.
