Project Description:
Develop a model of care for the DD population in Marion County that will increase access to healthcare services. This effort will develop a health home model that has many characteristics of a patient-centered health home but is customized to meet specific needs of low-income patients with DD who with chronic medical conditions, particularly those with co-occurring conditions. Failure to access needed primary care services can significantly impact people with DD, especially those with chronic disease. Objectives are to 1) emphasize prevention, lifestyle, and behavior change for individuals with DD and 2) provide earlier intervention and treatment to avoid high-cost emergency room visits/hospitalizations. outcomes: reduced inpatient hospitalization and emergency room visits and improved health care outcomes for participants. Additional indicators will be patient ahderence to care plan and ability to meet care plan goals.
