Project Description:
Community of Practice for Supporting Families Expansion and Continuity/Sustainability Goals:
Participate in project management monthly team meetings by phone
Develop, monitor and update implementation time lines for project deliverables
Develop communication strategies with project management team, project experts and interested
others
Participate in the development and implementation of Community of Practice (CoP) with
eleven (10) expansion states
Develop and refine the Model Framework for state systems
Work with project management team to develop self-assessmenttool to be used by the
participating CoP states
Work with states to develop their individual state work plan including identification and
training
of state facilitators
Attend state planning meeting with NASDDDS co-project director with each state in year one of
the project
Attend state implementation meetings as negotiated with the project team and requested by
states in years two and three
Attend CoP annual meeting each of the three project years
Plan for and participate in a minimum of bi- monthly CoP calls with the ten states
Expand web site for project to include CoP expansion states and general information updates
Assess states' progression and provide technical assistance as needed
Assist states in determining their evaluation or outcome measurements, if requested
Assist states in summarizing progress and findings from Community of Practice Activities
Assist with material preparation for CoP calls and meetings
Coordinate webinars on a quarterly basis and bi-monthly all-state CoP calls
Conduct an annual CoP meeting in Kansas City, Missouri for all CoP states inclusive of all
related activities necessary to coordinate the conference, such as materials, hotel arrangements,
audiovisual, outreach and registration
Offer LifeCourse Ambassadors Educational Training (six-part series) and National LifeCourse
Tools Workgroup for up to five state members
