Project Description:

The purpose of this contract between One Community One Family and Indiana University, Center for Collaborative Systems Change (CCSC) at IIDC is to evaluate the One Community One Family Safety PIN (Protecting Indiana Newborns) funded by the IN State Department of Health through a grant to One Community One Family, in partnership with Choices Coordinated Care Solutions. One Community One Family received funding to develop and coordinate implementation of a care coordination in partnership with Choices to support new and pregnant mothers with substance misuse issues to identify and access appropriate services in an effort to reduce infant mortality and increase healthy birth outcomes.
