Project Description:
The purpose of the project is to Conduct a needs assessment to determine current and future needs of St. Louis County residents with DD and produce a report with findings and recommendations for action. The needs assessments will result in a prioritization of needs and provide recommendations that will guide the development and provision of current and future services and supports provided by the Productive Living Board (PLB). The needs assessment will be guided by the following objectives: 1. To form a leadership advisory group to guide the implementation of the needs assessment and develop recommendations for the PLB. 2. To determine the current and future numbers of individuals with IDD, in St. Louis County and identify population trends. 3. To identify and review existing state and local data sets and assessments and correlate results with needs assessment findings. 4. To identify the current and future service needs of individuals with IDD who are over the age of 18. 5. To assess the capacity of the formal and informal service system to meet current and future service needs of individuals with IDD. 6. To identify service gaps and prioritize services and supports most needed by individuals with IDD. 7. To improve the capacity of the Productive Living Board to strategically collect and use data on an ongoing basis that informs their investment decision making process. This needs assessment will assist the PLB improve current and future services by identifying current needs, as well as anticipated needs of individuals with IDD as they age or transition into adult services in the upcoming years.
