Project Description:

The project will accomplish the following initiatives to improve the healthcare for adults with I/DD living in Anchorage: 1) promote the development of a cadre of emerging healthcare providers who have special interests in the healthcare of people with disabilities; 2)enhance patient-provider communication which will improve assessment of the healthcare needs of adults with I/DD; 3) facilitate providers referral for appropriate services; 4) empower adults with I/DD, their caregivers, and families with resources for health promotion activities.
