CATCH Planning Grant - Improving Transitions for Youths with Special Health Care Needs
Project Description:
The project will bring together key participants in facilitating transitions to adult life for Youth with Special Health Care Needs (YSHCN). Together, in a coordinated and dedicated effort, it is possible to improve the transition process and help to empower adolescents with SHCN to achieve self-care and independence. Funding of this project will allow the input of YSHCN and their families as well as their potential health care providers toward addressing barriers and illuminating preferences in the transition process. The establishment of a Coalition among agencies developing transition policy will prevent parallel development but rather a collaborative and interactive process.
