Project Description:
The purpose of this project is for CDD to serve as a State Expert Advisor to two local communities participating in the Reaching People with Disabilities through Healthy Communities national pilot project with NACDD and the CDC Disability and Health Branch. CDD will work with two local community partners (located in Woodbury and Carroll county Iowa) by providing the necessary state-based technical assistance in the ongoing implementation and evaluation of the project, participating in virtual and in-person project-related training with NACDD and CDC for the project period, and developing sustainability, technical assistance, and engagement plans with Community Partners.
