Project Description:
NEED: For many, the current health care model includes activities that occur within the clinical setting. However, research and practice has demonstrated that social services such as the need for transportation, utilities, housing, non-violent environments, and food security overshadows a patient's capacity to respond the the medical treatment fully. GOAL: This CMS-funded study is a randomized trial in which the treatment group receives additional social service patient navigation. The WVU CED will provide the navigational services. Once identified, patients will be connected to a local provider for that social service. The navigator will follow the patient over a particular period of time to assess the outcome of the referral.
