Project Description:
Goal: Provide state-level technical assistance, training, and support to pilot teams that will include Community Mental Health Service Providers (CMHSPs or CMHs), Prepaid Inpatient Health Plans (PHIPs), Intermediate School Districts (ISDs), people with intellectual/developmental disabilities (I/DD), and family members to address the issues with consistency, availability, and access to the tools and supports that people with I/DD need to control their services and live self-directed, self-determined lives. Project Objectives: 1) Increase the number of people who use arrangements that support Self-Determination by educating and supporting pilot teams that include CMHSPs, PIHPs, ISDs, people with I/DD and family members, to develop and refine their process for setting up arrangements that support Self-Determination. 2) Improve the ability of CMHSPs, PIHPs, ISDs, people I/DD and family members to support and arrange Self-Determination. 3) Education individuals with I/DD, including those with high and complex needs, their family members and allies to increase their knowledge of person-centered planning and self-determination to increase the demand for Self-Determination. 4) The Project Advisory Team in collaboration with MDHHS/BHDDA will develop requirements and standards that ensure consistent, quality person-centered planning that results in people with I/DD directing their own supports and services and changes in MDHHS/BHDDA polices, procedures, guidance, and technical assistance. Expected Outcome: Individuals with I/DD and their families are aware of the their rights and able to direct the supports and services they are entitled to by law across their lifespan, including education, adult services and long-term care supports.
