        Project Description:
        The purpose of the Utah Disability Health Program (UDHP) is to improve the quality of lives (especially through inclusion and accessibility) and reduce health disparities for people with mobility limitations (ML) and  /or intellectual disabilities (ID) in Utah. The UDHP will be implemented in the Utah Department of Health, Bureau of Health Promotion (BHP).
Overarching strategy: Improve accessibility and reduce disparities for people with disabilities in public health programs.
I. Project Period Outcome: Improved sustainability of programmatic improvement
Outcome Measure: Improved organizational capacity
Strategy: Develop internal and partner capacity
II. Project Period Outcome: Improved sustainability of policy, systems and environmental improvements
Outcome Measure: Improved collaboration with partners
Strategy: Enhance and expand sustainable partnerships
III.Project Period Outcome: 1) Improved data collection methods, 2) Increased availability of health promotion resources, tools, and inclusion strategies.
Outcome Measure: Improved monitoring of health and health care utilization
Strategy: Assess, develop, and disseminate health promotion resources, tools, and inclusion strategies
IV. Project Period Outcome: Increased availability and use of health promotion resources, tools, and inclusion strategies
Outcome Measure: Improved evidence base for health promotion programs
Strategy: Deploy evidence-based health promotion  programs adapted for people with disabilities (to be initiated by end of year 3)
V.Project Period Outcome: Increased sustainability of evidence based programs
Outcome Measure: Improved evidence base for health promotion programs
