From Oct 1, 2016 Sept. 30, 2017, through the Indiana Institute on Disability and Community (IIDC), the Trustees of Indiana University agree to deliver the following services and deliverables to support ISDHs Division of Chronic Disease, Primary Care and Rural Health implementation of Phase VI of the Chronic Disease Related Health Disparities Affecting People with Disabilities in Indiana project: 1) continue to convene and support the ongoing advisory role of the statewide Task Force on Disability and Health, 2) conduct an environmental scan of extant evidence-based health promotion programs in Indiana of potential benefit to persons with disabilities, with a focus on their actual or potential inclusivity for the target population, 3) select one or more evidence-based programs to cooperate in the piloting of an effort to more effectively reach people with disabilities for participation in extant programs, 4) employ information from the pilot to inform and encourage greater inclusivity among providers of such programs throughout Indiana, 5) develop and conduct a webinar on the effective gathering and utilization of data for community health assessment purposes in Indiana and, 6) employ elements of the Health Matters model to pilot an Inclusive Community Advocates project that brings together people with disabilities, older adults, and health/disabilities providers to achieve greater inclusion of people with disabilities in community-based health prevention and health care programs.
