Project Description:
In Maine's second-oldest, most rural, and least healthy county, the Piscataquis TiP Collaborative (PTC) will create a sustainable and comprehensive resource/referral network that helps people with chronic health conditions avoid out-of-home care. With a diverse and robust volunteer and professional workforce, this network will be nimble enough to respond quickly to all referrals with a person-centered approach, assisting individuals and caregivers with assessment, planning, and referrals. Through a collective impact process, the PTC will increase the integration and coordination of existing resources and build capacity in the following areas: health-related transportation; volunteerism; home-based services; caregiver support; and community engagement. The Center for Community Inclusion and Disability Studies will conduct Person-Centered Planning training and client planning sessions to increase awareness and utilization of this model throughout the Piscataquis region.
