Project Description:
1. Dr. Larry Yin consults with nurses on medical issues involving clients who have recently been moved out of the developmental center and into 853 homes. These consultations center on medical care needs, best practices for health monitoring, and recommendations for subspecialty medical care. Dr. Yin also provides regular training to SGPRC nurses regarding multiple medical conditions frequent in adults with DD (anemia, asthma, diabetes, immunizations for adults, infections frequent in those with DD, etc.) to build capacity when they see our clients in the home and hospital settings. Dr. Niru Madduri participates in medication evaluation and follow-up for young adults with DD and disruptive behaviors. She works with a psychopharmacologist and behavior a list attempting to reduce involuntary/voluntary psychiatric hospitalization in this population. At the Neurology Clinic, Dr. Arthur Partikian sees adults with DD and neurologic issues(epilepsy, movement disorders, etc) for consultation, medication management. 2. Dr. Larry Yin reviews all deaths at the regional center and direct staff to follow-up on potential care issues as a process improvement /prevention strategy to improve the health of adults in our system. From this activity, public health strategies have been developed to inform parents/care providers on immunizations for adults, choking/feeding issues that develop in our population, health surveillance strategies, and continue to develop new strategies and information pamphlets to prevent deaths. Direct outcomes include decreased choking related deaths, decreased accidental drowning, decreased death from heat exhaustion, better surveillance for cancer, liver disease, heart disease and sleep apnea. There has also been improved access to oral health and instituted better hand hygiene in ICFs, CCFs to prevent spread of infection. 3. Conduct wellness fairs for adult clients and offer health screenings. 4. Through this work with Adult Health through SGPRC, life span outcomes have been improved and resulted in increased access, capacity, and better hygiene.
