Project Description:
The Developmental Disabilities Institute (DDI) at Wayne State University will collaborate with The Arc Michigan to evaluate the Dignified Lifestyles Project by completing the following activities: Design a revised person-centered planning interview tool and record review tool based on the feedback from MDHHS and PCP requirements in the HCBS Final rule. Train the PCP Site Reviewers on the use of the new tool (two trainings: Pilot Sites and FY 2017 Sites). Compile the data from the new tools. Prepare a report on the data. Participate in meetings as required.
