Project Description:
The District of Columbia is engaged in multiple systems-integration initiative that will be leveraged to improve, strengthen, coordinate and transform the Long-Term Supports and Services (LTSS) system. Since October 1, 2015, the District's Health and Human Services agencies, led by the Department of Disability Services, are implementing a three-year plan, to create a "No Wrong Door" system of access to LTSS for the city's diverse populations. The project is known as the No Wrong Door(NWD) initiative. NWD is a three-year implementation grant funded by the federal government, HHS,Administration for Community Living.
Goals: (1) To transform current District's health and human services programs and processes into a single NWD system for LTSS for diverse populations and payers. (2) To create a LTSS that is person-centered and which all staff possess core competencies that facilitate connection to formal and informal LTSS, regardless of where people across the age span connect to the service system.
The Initiative is designed to: (a) Create a sustainable infrastructure that enhances consumer choice and control; (b) Result in a consumer-driven, efficient and cost-effective system of LTSS; and (c)Explore linkages to family, community and technological supports and services prior to discussions about publicly-funded LTSS. Outcomes include streamlined integrated referral, intake, and eligibility determinations; uniform cross-system person-centered service-delivery approaches; user-friendly resource portals to connect people to public and private LTSS; culturally/linguistically competent approaches to engage and serve stakeholders; and formal agreements to promote sustainable change.
