Project Description:
The overall goal of the Mailman Centers Family Navigator Project is to ensure that children and their families receive needed services by (1) creating in-person and on-line tools to help families understand their childs needs and available services; (2) implementing a system of care that educates and connects families to need services; and (3) enhancing the current UM quality improvement (QI) process to include measures of family-centered care, including identification of and connection to needed services.
