Project Description:

Goal: Improve care coordination for children with medical complexity to decrease cost and improve outcomes. Description: Employing small cycles of change using PDSA (plan, do, check act) methodology Family Engagement Encouraged Patient Enrollment and Consenting allows TRUVEN to obtain Medicaid claims data to determine new payment model for this population Change Concepts: Registry, Dynamic Care Teams (DCT), Access Plans, Care Plans, Transition Locus of Care Management
