Project Description:
The Center for Development and Disability is designated by the New Mexico Department of Health as the statewide entity to administer the Supports Intensity Scale (SIS) to individuals on the DD Waiver whose Individual Service Plans are approaching their three-year anniversary. The SIS is the first step in a four step process of determining a package of services for DD Waiver participants. SIS results are sent to the Health Department, where a proposed service package is assigned to the individual. The Individual Development Team (IDP) reviews the package and makes changes as needed. Finally, the entire package is reviewed by the Outside Reviewer, which makes the final determination regarding services. The CDD also provides training and outreach on use of the SIS throughout the state.
