MDHHS Home and Community Based Transition Project (HCBS)

The Michigan Department of Community Health (MDCH) has developed a transition plan for how Michigan will address the recent Center for Medicare and Medicaid (CMS) rule change on Home and Community-Based Services (HCBS). DDI will assist MDCH to assess its status for compliance, assist with transitions to compliance, and provide technical assistance during the transition process. The project objectives are: 1. Conduct onsite verification visits with sub-sample of the HCBS Habitation Support Wavier respondents 2.Summarize finding from onsite verification visits and report result MDHHS 3. Conduct analyses from the respondent data 4. Report project findings.
