Project Description:
Results from the National Survey of Children with Special Health Care Needs indicates that only 40% of youth with special health care needs in the US receive adequate services for transitioning from pediatric to adult health. The TEAM project will: 1) develop and implement innovative medical transition resources to aid youth with DD or SHCN, parents, educators, and their health care providers in three geographic regions of ND; 2) Create GoAnimte modules for youth and parent participants along with BlackBoard technology for all participants including their current medical provider will be developed and disseminated; and 3) give participants a better understanding of what is included in a medical transition and will have begun the process with their medical team to aid in a successful transition.
