Project Description:
The Indiana Institute on Disability and Community (IIDC), Center on Aging and Community, the Trustees of Indiana University agree to deliver the following services and deliverables to support the Indiana State Department of Health (ISDH) Division of Chronic Disease Prevention and Control (CDPC) implementation of Phase IV of the Chronic Disease Related Health Disparities Affecting People with Disabilities in Indiana project: 1) digest and disseminate results of the research and recommendations of the Disability and Health Task Force; 2) engage and educate a broad audience of organizations and agencies in a position to reduce health disparities and 3) secure commitments from such organizations to develop, undertake and monitor plans to build their capacity to reduce such disparities, 4) continue to seek advice and input from the Disability and Health Task Force
