Project Description:

The Michigan Department of Community Health (MDCH) has developed a transition plan for how Michigan will address the recent Center for Medicare and Medicaid (CMS) rule change on Home and Community-Based Services (HCBS). DDI will assist MDCH in designing and implementing the HCBS setting rule compliance. This will include: 1. To design and refine the assessment tools and assessment methodology. 2. To assist in assessment, data entry, and data analysis. 3. To assist in on-going monitoring of plan implementation and compliance.
