HEALTH - Health Promotion for Ohioans with Disabilities: Preventive Care, Emergency Preparedness, and Access to Health Care

Project Description:
The program's goal is to increase capacity and action to promote and maximize health, prevent chronic disease, improve emergency preparedness and increase quality of life among Ohioans with disabilities. There are seven goals including 1: Enhance Program Infrastructure and Capacity, where the Ohio Disability and Health Program (ODHP) will form a partnership between the two Ohio UCEDDs, Ohio Department of Health (ODH) and the Government Resource Center (GRC). A Disability Community Planning Group will be recruited to guide the program. Goal 2: Improve State Level Surveillance, will be accomplished GRC leading in quantifying and measuring conditions/health issues among PWD in Ohio. Goal 3: Increase Awareness of Health-related Disability Policy Initiatives, allows ODHP to develop policy initiatives that improve PWD health in Ohio. We will partner with ODH Division of Prevention, Ohio Department of Aging and Centers for Independent Living to achieve Goal 4: Increase Health Promotion Opportunities for PWD, by conducting health promotion demonstration projects with PWD and train-the-trainer sessions to build capacity in the disability community. ODHP will provide sensitivity trainings to public health/health care provider workers. For Goal 5: Improve Health Care Access for PWD, we will work with the Ohio Association of Community Health Centers to increase access to Ohio Federally Qualified Health Centers. Goal 6: Improve Emergency Preparedness among PWD, will involve Ohio EMA and Office of Health Preparedness appointing PWD planning committees, revising the Ohio EMA plan to address PWD needs, increase PWD having emergency plans, train first responders on PWD needs and improve emergency shelter access. For Goal 7: Evaluate Program, ODHP will assess change in prevalence, utilization, quality of life indicators, health equity standards and program effectiveness. ODHP will provide disability and health related technical assistance for businesses, organizations and PWD in Ohio.
