Project Description:
The project addresses a need for community management of chronic conditions, including developmental disabilities, and assures access to specialty care using tele health. The project involves collaboration with a Federally Qualified Health Center, and provides community based care for children with developmental disabilities, asthma, diabetes, and sickle cell disease.
