Project CARE/Detroit-Wayne County Community Mental Health
Project Description:
1) DDI will participate on the Services Management Committee to facilitate and promote communication among the project partners.
2) DDI will participate in an initial review ("Gap Analysis") of the Agency's service delivery system.
3) Based on the Gap Analysis, DDI will help identify deficiencies and need for remedial action.
4) DDI will help develop and implement and utilize survey and monitoring tools.
5) DDI will help develop information and training materials.
6) DDI will participate in weekly meetings to communicate plans, progress, issues, and develop a set of goals and workplan.
