As part of the community development initiatives at CDR, the Supported Community Living Project was developed to assist people with disabilities in transitioning from their family homes or congregate living environments to their own homes. Resulting from this initiative, CDR staff and collaborating partners have established a systematic assessment and planning process to secure housing, funding and individualized support services for people enrolled in the project. Relationships have been established with several local housing authorities and the State Housing and Finance Authority to enhance opportunities for people with disabilities who have very low incomes. Community based technical assistance efforts worked to establish a housing bridge funding program with SCDDSN to support people prior to transitioning to Section 8 voucher funding. The Supported Community Living Coalition, comprised of provider agencies and SCDDSN, was created in January 2006 to address funding barriers to supported living. During the year, the funding concerns have been alleviated and the coalition has successfully addressed other systemic barriers to increase individualized supports. In addition to the hands-on technical assistance and mentoring , a comprehensive and competency-based curriculum for Supported Community Living Coordination (a form of support brokerage) has been developed. In partnership with SCDDSN and other local agencies, training on this curriculum will be provided statewide to individuals receiving supports, family members, support staff, and service coordinators and other professionals.
