The Institute for Community Inclusion at the University of Massachusetts Boston in partnership with Children?s Hospital Boston and the Parent Advocacy Coalition for Educational Rights (PACER), and working in affiliation with the Association for Maternal and Child Health Programs and the American Academy of Pediatrics responded to Announcement HRSA-10-096 to create a new National Center for Community Based Services for Children and Youth with Special Health Care Needs under a cooperative agreement with the Maternal and Child Health Bureau. This cooperative agreement addresses one of the six core outcome measures for success in serving CYSHCN and Youth with Special Health Care Needs, namely that ?services for children with special health care needs and their families will be organized in ways that families can use them easily.? The center has and will continue to collect and disseminate information about evidence-based best practices in community based service provision that supports the objectives of MCHB in reaching the national objectives for children with special health care needs. The center?s purpose and current accomplishments include: ? Conducting extensive analysis of the data pertaining to ease of use of community-based services from available national datasets, in particular, the National Survey of Children with Special Health Care Needs (NS-CSHCN). ? Developing, supporting, and sustaining an Action Learning Collaborative (ALC) for Title V /CSHCN Directors, family and health care practitioners. ? Integrating continuous quality improvement efforts in to the collaborative work with PACER. ? Conducting case studies to identify successful models of easy to use community-based services for Latino families with CSHCN. ? Disseminate products to a wide range of stakeholders through media and channels most familiar to the target audiences. ? Participating and providing support for Division conferences, grantee meetings, and other expert panels. ? Maintain a public website as a repository of information on community based services for CYSHCN. An accessible public website has been created (www.communitybasedservices.org). In the past year, we had approximately 3000 individuals access the website and 2000 individuals download documents from the site.
