Project Description:
Nebraska proposes to implement a consumer-directed service delivery system across populations – adults and children with physical disabilities, developmental disabilities, behavioral health needs, medically-complex conditions, as well as issues related to aging. Consumer choice and risk must be defined and incorporated into service delivery mechanisms, regulations, quality assurance issues, and practices. A Consumer Task Force, in partnership with agency and program staff, will collaborate to design and implement effective and enduring improvements in a culturally competent, community long-term care service and support system. They will work together to identify quality measures and help design and implement a sustainable quality management system that monitors the efficacy of services in a manner that meets consumer’s expectations and preferences.
