Project Description:

The primary purpose of this contract is for the identification of Self-Advocacy (SA) groups in Arizona. Specifically, the IHD/AzUCD will develop and conduct a survey to identify SA groups in Arizona with an emphasis on local, statewide and national groups in each Arizona county. The survey will include information about active membership, products, community partnerships, impact, structure and resource. The data from the survey will be compiled and submitted to the ADDPC in a report.
