Project Description:
The Community Transformation Grant (CTG) program supports state and local governmental agencies and community-based organizations in the implementation, evaluation and dissemination of evidence-based community health activities in order to reduce chronic disease rates, prevent the development of secondary conditions, address health disparities, and develop a stronger evidence-based of effective prevention programming. The overarching purpose of this program to prevent heart attack, stroke, cancer, diabetes and other leading chronic disease-related cuases of death or disability through a variety of "policy, environmental, programmatic and, as appropriate, infrastructure" interventions to promote healthier lifestyles. On behalf of Southwest Indiana Healthy Communities Partnership, the Welborn Baptist Foundation has been awarded a Small Communities CTG aimed at improving the health of specific populations in a seven-county region of Southweset Indiana. The evalation project willb e conducted by a collaborative team from Indiana U nivesity, which combines evaluation exterise from CELL and the Indiana Prevention Resource Center (IPRC). A comprehensive process and outcome evaluation plan will be aligned with the project to ensure that data collection efforts conbtribute to the ongoing implementation of the plan and the assessment of relevant outcomes. In addition, the evaluation team will collect supplemental contextual data to address questions of efficiency and understand stakeholder perceptions of the process.
