Project Description:
Early childhood transition, from early intervention to other services including preschool special education, is an identified priority in both New Mexico's Part C and Part B 619 programs. Through the coordination of transition across the state, and support for community teams composed of school districts, early intervention programs, childcare, Head Start, and other community representatives, the project works to ensure that all children and families will experience a smooth and effective transition as a result of collaborative intentional community planning, statewide training and local coach ensure on-going support.
