APS Safe Schools - Home Visiting
Project Description:
Objective: Enroll families, up to 55 young children (ages 0-3), from the Helen Cordero Primary School Community, for Parents as Teachers ("PAT"0 training through home visits under the Safe Schools/Health Schools Initiative.
A.) Home visits with families in the Helen Cordero Primary School Community
B.) Program introduction to school community, including school and other stakeholders.
C.) Recruit and hire 1.5 home visitors to provide direct service as per PAT model.
D.) Train home visitors in the PAT model.
E.) Recruit and enroll up to 55 families to participate in the PAT program.
F.) Maintain all program records, including family
