Project Description:

SCOPE OF WORK Develop a joint collaborative of self advocacy leaders that will work closely with the New Mexico Developmental Disabilities Planning Council?s Center for Self Advocacy to support and strengthen a statewide self advocacy network led and maintained by persons with developmental disabilities. 1) CONTRACTOR will provide the following: a. Provide an action plan that includes a description of each activity and its outcomes as it relates to self advocacy awareness, disability awareness, network building or leadership. b. Provide a time line for each activity and its outcome. c. Provide a plan for collaboration with the DDPC. d. Provide in detail and document outcomes to the DDPC to Tony 6/27/12
