ADRC/MFP Nursing Home Transition and Diversion initiative
Project Description:
The overall goal of the project is to increase the number of adults with disabilities and older adults successfully transitioning back to the community from nursing facilities by building technology and education infrastructure and creating capacity to implement MDS.
IHD provides support for a number of capacity building components. These include development of curricula for the planned training initiatives, working with marketing consultants to develop materials, and conducting project evaluations.
Expected outcomes Obj. Table 1: Targets
1 20% increase in follow-up provided those expressing interest in transitioning
2 28% of individuals on the MDS express an interest in transitioning (current 18%)
3 10% increase in individuals with a guardian expressing an interest in transitioning
4 20% increase in successful transitions from referrals on the MDS 3.0 Section Q.
5, 6 20% increase in correct referrals for community transitioning.
