Project Description:
This Addendum is pursuant to the National Performance Measures for Maternal Child Health which expire 10/31/2012
31 PERFORMANCE MEASURE: The degree to which grantees have assisted States and communities in planning and implementing comprehensive, coordinated care for MCH populations.
Goal: Improve the Health Infrastructure and Systems of Care (Assist States and communities to plan and develop comprehensive, integrated service
41 PERFORMANCE MEASURE: The degree to which grantees have assisted in developing, supporting, and promoting medical homes for MCH populations.
Goal: Ensure Quality of Care (develop and promote health services and systems designed to improve quality of care)
The University shall:
1. Develop on-line training modules related to Medical Home and Health Literacy for early childhood providers. Activities will include:
a.) Convening stakeholder group for guidance and input in module development;
b.) Develop timeline and objectives with input from stakeholder group;
c.) Gather and review existing materials relative to Medial Home and Health Literacy for early childhood providers;
d.) Identify gaps in current resources which will be addressed through module(s);
e.) Pilot module(s) with potential users for final refinement.
2. Participate in financial audits when requested two (2) weeks in advance by the Department.
3. Submit the invoice for each month by the last day of the following month. Each monthly invoice is to be for an equal portion of the total Addendum amount.
4. Ensure diversity of programs and structure. Ensure that programs offered meet federal cultural and linguistic access standards to better serve the target population.
General Provisions
5. Performance will be monitored and evaluated by periodic on site work reviews, review of monthly data reports, and scheduled consultations with MCH.
