Project Description:
"AZ Community Living NOW" will address the DDPC's area of emphasis goal #1 of increasing opportunity and choice in housing for individual with DD and their families through a comprehensive education program on Arizona options for affordable and accessible housing (Objective 2). Too many times, individuals with DD and their families believe the only options they have are agency supported residential services or living at home. The Sonoran UCEDD will challenge families to think creatively as they learn about current options and ideas for community living. We will develop a manual for families and individuals with disabilities and their providers on affordable and accessible housing, different housing options, legal concerns, and how to develop a sound plan for where to live and who to live with. The manual will be available in hard copy and through a searchable web module with audio component. The manual and web module will cover the following subject matter crucial to individuals and families contemplating community living options: 1. Community Services and Supports; 2. How to discuss and plan for housing transitions; 3. Accessible Housing and Home Modification; 4. Shared Living, Co-ops and Consumer/family housing; 5. Special Needs Planning; 6. Financial Education and Planning and 7. Renting and Owning your own home. Dissemination of the manual will be targeted to underserved/unserved populations utilizing the network of disability and community organizations in the State. The Sonoran UCEDD will assess the success of the project both by determining knowledge gained and satisfaction with the manual and web-site as well as a small sample chosen for a follow up survey to assess how the information was used by the family to assess affordable and accessible housing.
