Project Description:
The purpose of this project is to transition individuals from hospitals and nursing facilities to community settings. The project will review and consider individuals currently institutionalized, including the elderly, medically-fragile children, and youth and adults with disabilities who express a choice of community living.
This five year project renewed annually originally began in 2007. It is funded by Centers for Medicare and Medicaid Services Money Follows the Person Rebalancing Demonstration grant. The project is a collaborative effort between the Department of Human Services (the state Medicaid agency) and its community partners. The community partners will serve as members of a Stakeholder group to plan the successful implementation of the project. The Center on Disability Studies at the University of Hawaii will be a partner in the pre-implementation phase and in the evaluation of the project.
