Project Description:
The overall goal of the project is to "establish a coordinated system of care in Kansas to enhance health care access, services, and transition supports for Kansas youth with special health care needs and their families". Directed by a youth advisory group and a key stakeholder advisory group, the project addressses the needs of youth with special health care needs in being able to transition from pediatric to adult medical care, access coordinated services through a medical home, and achieve their post-school outcomes through a collaborative effort involving the education and medical communities. Major activities include the development of an on-line interactive health care transition curriculum for use in high school programs, pre-service training within education programs for medical and related service personnel, model development and pilot testing of a health care transition model, and training and information dissemination to families and professionals. A significant feature of this project is involvement and collaboration with statewide disability organizations, medical personnel, families, and youth.
