Project Description:

Patient Centered Medical Home Initiative (PCMHI) is a demonstration program that seeks to implement patient-centered medical home models in a limited number of primary care practices and community health centers. The ?technology support? component of the initiative includes a project website, online courses, discussion forums and webinar capabilities. The website and online content provides a centralized place for information to ?live? for access by the large numbers of practices involved in the initiative. The online communication and education support mechanisms will be designed to extend the reach of the project as well as strengthen its impact.
