Project Description:
Project Context. The Center for New Community Opportunities is a national training and technical assistance program of the Independent Living Research Utilization (ILRU) organization. Its mission is two-pronged: 1) to promote community-based alternatives to institutionalization and successfully transition youth with disabilities into community life, and 2) to facilitate adults with disabilities transitioning from nursing homes to community living. The NCO Center also addresses how these services can best be provided to underserved populations, particularly in isolated rural areas. The Center accomplishes this mission by training and equipping Centers for Independent Living (CILs) to develop community-based service programs. Project Focus. The CPD will provide training and technical assistance to CIL staff to share promising practices from other home and community-based service programs that will help individuals with significant disabilities move from institutional settings to community-based living and prepare transition age youth to live independently, including postsecondary education and employment. A secondary focus will be on nursing home transition into community living. Project Objectives/Activities. The CPD-NCO project activities include: 1) Curriculum development and delivery of on-location conference training for CILs on Youth Transition. 2) Development and delivery of live and on-demand Webinars for CILs on Youth Transition. 3) Development and delivery of an online course for CILs on Youth Transition. 4) Development and delivery of on-demand video training for CILs on Nursing Home Transition. 5) Provision of coaching and technical assistance for CILs on Youth Transition. 6) Development of Resource Lists, and Spanish version of NCO materials. 7) Development and maintenance of CIL Blogs: Youth Transition and Community Alternatives to Nursing Homes. 8) Provide formative evaluation of overall T/TA for NCO projects
