Project Description:
Metropolitan Community Provider Network (MCPN) is a federally qualified health center providing primary care and other community services for underserved, uninsured, Medicaid and Medicare populations. The Potomac Street Health Center (PSHC) serves people with developmental and other disabilities from adolescence through geriatric care, and adults age 55 years and up. The clinic operates on a Medical Home Model and offers case management and health education services as well as house calls for people with disabilities and elderly individuals. JFK Partners provides a Family Nurse Practitioner who provides primary health care to patients, most of whom have disabilities or chronic health conditions.
