Mount Pleasant Monitoring Project
Project Description:
The goal of the Mount Pleasant Center Monitoring Project is to monitor and evaluate the placements for individuals who formerly living at Mount Pleasant Center (MPC) to the community assuring that all placements are safe and successful. This project will be achieved through the implementation of the following activities:
A. Conduct on-site visits to all persons with developmental disabilities placed from MPC since the fall of 2007. This process will include meeting with the individual, their family/guardian and other key stakeholders at one month following discharge and six months from date of the first visit. Additional and/or intermediary visits will be conducted (if necessary) based on the scheduled visit findings.
B. The Developmental Disabilities Institute (DDI) will collect individual baseline data utilizing a monitoring tool developed by DDI to note issues of health and safety and to report on the quality of life.
C. Monitoring will include a record review to ensure the implementation of the person centered planning process and the MPC discharge plan.
D. DDI will report findings on a quarterly and annual basis to the MPC Community Transition Leader and the Michigan Department of Community Health. These reports will update data for persons moved from MPC to the community, make comparisons to the baseline data, and identify significant trends or areas of improvement.
