Wayne County Family Support 360 Expansion Project
Project Description:
Project goal: To implement the Detroit Family Support 360 One-Stop Center throughout Wayne County to assist low-income, minority families with a family member with a developmental disability to preserve, strengthen, and maintain their family units.
Project objectives:
1. Establish and maintain the Detroit Family Support 360 One-Stop Center.
2. Recruit and serve at least 50 families per year.
3. Facilitate sustainability of the Detroit Family Support 360 One-Stop Center.
4. Evaluate the project process, effectiveness, and outcomes.
Expected outcome: The Detroit Family Support 360 Project will preserve, strengthen, and maintain the family unit, while making sure that the individuals with developmental disabilities receive ample opportunities to exercise greater choice and self-determination.
Eligibility Criteria:
The family must meet the following criteria:
- Include a child (under the age of 18 years old) with a developmental disability
- Reside within Wayne County, Michigan
- Be underserved and/or unserved by the family support service system
Description of Project Participation: Each family will be assigned a Family Support Navigator to support their pursuit of family support services. The Family Support Navigators will assist the family to identify the family's needs, identify possible resources of support, and collaborate with the Detroit Family Support 360 Partners to connect families to services and supports.
