Family to Family Partnership
Project Description:
The A.J. Pappanikou Center on Disabilities, in collaboration with the CT Family Support Network and the CT Family Support Council, PATH Parent-to-Parent, FAVOR, Inc. and the CT Department of Public Health is requesting $287,100 over three years to expand and implement the Connecticut Family-to-Family Health Information Network.
1. Organizational Setting: This is an unique collaborative proposal representing the collective knowledge and resources of a variety of parent and provider organizations. The project will be administered by the A.J. Pappanikou Center on Disabilities, UConn Health Center. The project will be implemented in collaboration with the Connecticut Family Support Network and the Connecticut Family Support Council; FAVOR, Inc; PATH Parent-to-Parent; and the Connecticut Department of Public Health. The A.J. Pappanikou Center supports a number of projects affecting children and adults with special health care needs, including CT Family Voices. The CT Family Support Network is a well established network of parents and parent support groups across the state. The Family Support Council is a parent professional partnership created in CT state statute to coordinate access to agencies by agencies that administer or fund family support services to families raising children with disabilities. FAVOR is a statewide family-directed advocacy organization for children's mental health. PATH Parent-to-Parent is a statewide parent run agency providing one-to-one support to parents who have a child with special needs. The CT Department of Public Health administers the Title V CSHCN program and the statewide Medical Home project in the state.
2. Goals: This project will build on the accomplishments of the existing CT Family-to-Family Health Information Network presently located at FAVOR, Inc. The project will provide support, information, and advocacy to families and caregivers in Connecticut who have a child with special health care needs, including children with behavioral health needs. The primary outcome of this project will be to assist families and providers in navigating the public and private helath care financing service delivery systems and to develop appropriate strategies and policies to improve these systems. The project will achieve this through four goals: (1) Provide information on health care resources to families and caregivers of children with special health care needs through the provision of direct support and advocacy to families and caregivers. (2) Build the capacity of existing networks to support and inform families by mentoring parent leaders across the state. (3) Develop policy responses for issues identified by families who have a child with special health care needs in Connecticut. (4) Develop family/professional partnerships to meet the support and care needs of children with special health care needs and their families by providing support to parents to participate in a number of advisory councils, committees and projects.
