Project Description:
The Adolescent Health Transition Project (AHTP) provides information to help smooth the transition from pediatric to adult health care for youth with special health care needs. The project works with the Washington State Department of Health, Children with Special Health Care Needs Program to examine issues related to health care needs for youth and young adults and to improve the transition into adult health care. The project is working to improve preparation of youth in the public school system for increasing independence in care of their health needs. The objectives of this project also align with the goals of the Maternal Child Health Bureau: (1) Providers will have the knowledge and tools to provide care for adolescents transitioning to adult care, and (2) Families and youth will have the knowledge and tools to provide care for adolescents transitioning to adult care. The Adolescent Transition Resource Notebook, Transition Time Lines and Transition Tips booklets are among the resources available from the project for families, health care providers, school personnel and others to assist in a successful transition to multiple aspects of adult life.
